For many years there has been a debate as to which is the method of choice in treating patients with esophageal perforation. The literature consists mainly of small case series. Strategies for aiding patients struck with this disease is changing as new and less traumatic treatment options are developing. We studied a relatively large consecutive material of esophageal perforations in an effort to evaluate prognostic factors, diagnostic efforts and treatment strategy in these patients.

Management has changed dramatically over the years as new and varied treatments have become available. The treatment differs between countries depending on the availability of the various therapies. Overall, medical therapy is undoubtedly the commonest form of treatment, with surgery being used as second line therapy or for intractable symptoms. This paper provides an overview of reflux disease and the available treatments, as well as describing the most common surgical techniques, providing the practitioner with guidance as to when surgery may be a viable option.

Liver graft dysfunction can deteriorate to complete organ failure and increases perioperative morbidity and mortality after liver transplantation. Therapeutic strategies reducing the rate of graft dysfunction are of current clinical relevance. One approach is the systemic application of prostaglandins, which were demonstrated to be beneficial in reducing ischemia-reperfusion injury. Preliminary data indicate a positive effect of prostacyclin analogue iloprost on allograft viability after liver transplantation. The objective of the study is to evaluate the impact of iloprost in a multi-center trial.
